Official Rules
- NO PURCHASE NECESSARY: No purchase of any kind is necessary to enter or win. Making a purchase will not increase your chances of winning. Void where prohibited by law. Sweepstakes is subject to all applicable federal, state, and local laws.
- Eligibility: Citizen Health’s Rare Care, Everywhere Back-to-School Giveaway (the "Sweepstakes") is open to legal residents of the fifty (50) United States (plus Puerto Rico and the District of Columbia) who, as of September 1, 2024 are at least eighteen (18) years of age and have, or be a parent or legal guardian of someone who has, Okur-Chung neurodevelopmental syndrome (CSNK2A1). Participation constitutes entrant's full and unconditional agreement to these Official Rules and to Citizen Health's random drawing of the sweepstakes winners, which is final and binding in all matters related to the Sweepstakes. Winning a prize is contingent upon fulfilling all requirements set forth herein. Citizen Health reserves the right to verify the eligibility of the winner. All entries should be submitted in good faith. Members of the workforce of Citizen Health or of CSN2KA1 Foundation are ineligible to participate.
- Duration: Sweepstake period is fromSeptember 12, 2024 through 11:59 p.m. ET on September 30, 2024.
- Entry method: There is NO PURCHASE NECESSARY to participate. A purchase does not increase your chances of winning. To enter, visit https://citizen.health/csnk2a1 and complete all required fields of the onscreen registration form to receive one (1) entry into the Sweepstakes.* Individuals (or parents or legal guardians of a minor) diagnosed with Okur-Chung neurodevelopmental syndrome (CSNK2A1) who are already using Citizen Health will be automatically entered. Potential winner is subject to verification before any prize will be awarded. Limit one (1) entry per person/email address via the registration form. No information regarding entries, other than as otherwise set forth in these Official Rules will be disclosed. Entries must be received by 11:59 p.m. ET on September 30, 2024 to be included in the random drawing. Any attempt by any entrant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and logins, or any other methods will void such entries and that entrant will be disqualified. Use of any automated or computer system to participate is prohibited and will result in disqualification.
- Prizes: there will be 1 gift card awarded in a denomination of $250.
- Winner selection: All eligible entries submitted during the Sweepstakes Period will be entered into a drawing on or about October 1, 2024.. One winner will be randomly drawn for the prize. Odds of winning depend on the number of eligible entries received for the sweepstakes. Limit one prize package per individual. Citizen Health will randomly select the prize winner from among all eligible entries received during the Sweepstakes Period. Citizen Health's selections of potential winners are final and binding in all matters related to the Sweepstakes. Drawing will be conducted by Citizen Health, using RandomPicker.com. RandomPicker.com is an independent organization. Administrator's decisions are final and binding on all matters related to the Sweepstakes. The odds of winning a prize depend on the total number of entries received throughout the Sweepstakes Period.
- Winner notification: Winners will be contacted at the entry email address used to enter the sweepstakes. To claim the prize, winners must follow the instructions provided in the notification email and respond within ten (10) days. After the winner claims the prize, Citizen Health will respond back with the prize redemption instructions. Failure to timely respond will result in disqualification. It is within the discretion of Citizen Health to select alternate winners if the prize is forfeited but is not required to do so.
*For an alternative method of entry, mail a 3 inch by 5 inch note card to WeWork c/o Citizen Health, 400 Concar Drive, San Mateo CA 94402, including full name of entrant; statement that the entrant has, or is the parent or legal guardian of someone who has, Okur-Chung neurodevelopmental syndrome (CSNK2A1); mailing address; and valid email address. The note card with the required information must be received by Citizen Health by the deadline of 11:59 p.m. ET on September 30, 2024.
